Passports & Renewals
You will need to come to the Clerk’s Office to apply in person for a U.S. passport if:
- You are applying for a passport for the first time.
- Your previous passport is not in your possession, or lost/stolen.
- Your previous U.S. passport has expired and was issued more than 15 years ago.
- Your previous U.S. passport was issued when you were under 16.

What you will need to apply for a passport:
- Appointment
- Application Forms (Having your application completed in black ink before you come into the office will save you time.) If you make a mistake completing the application, please start over, information crossed out or scribbled over is not accepted.
- Proof of Citizenship
- Passport Photo
- Passport Fees (Department of State passport application fees will need to be paid by Check or Money Order made out to the US Department of State.)
- Acceptance Facility Fee and Optional 1-2 Day Delivery mail fees can be paid by cash, check, or card to the Cheatham County Clerk.
- If you are applying for a child, the child must be present.
Applications without proper documents can not be processed. Your Original Proof of Citizenship (Birth Certificate, Certificate of Naturalization, Birth Abroad) will be returned to you in a separate envelope from your passport. For Birth Certificates, be sure that it is the long form with the parent's full name listed. If you were born in TN, you can obtain a certified copy of your Birth Certificate at the Cheatham County Health Department. They are located by the Cheatham County Library at 162 John Mayfield Drive, Suite 20, Ashland City, TN 37015.
